Mobility EASY Card

Print and fill out the application and bring it along with a valid Florida Driver’s License or Florida ID with a Miami-Dade County physical address as well as proof of income to the Golden Passport Office located on the first floor of the Government Center Metrorail Station, or at any of the 311 Centers. 

Applications can only be submitted in person. All applications received are placed on a waiting list on a first-come, first-served basis. 

Make sure to bring:

• A valid Florida Driver’s License or Florida ID showing a Miami-Dade County physical address
• Proof of income
  • Previous Year's Tax Return (self-prepared Tax Returns not accepted)
  • Previous Year's IRS Tax Return Transcript
  • 1 month of recent Paycheck stubs (new applicants only)
  • Previous Year's IRS Notice of Non-Filing and Previous Year's IRS Wage and Income Transcript (if no tax return was filed)

Your Transit Mobility EASY Card must be renewed annually on or before the expiration date printed on the card.

You can renew by mail, email or fax by providing copies of the required documents and a signed application.

To renew in person bring with you:

• A current valid Florida ID or driver’s license with a Miami-Dade County physical address
• Social Security Card
• Proof of income
  • Previous Year's Tax Return (self-prepared Tax Returns not accepted)
  • Previous Year's IRS Tax Return Transcript
  • 1 month of recent Paycheck stubs (new applicants only)
  • Previous Year's IRS Notice of Non-Filing and Previous Year's IRS Wage and Income Transcript (If no tax return was filed)
• Your last-issued Transit Mobility EASY Card

Replace your lost, stolen or damaged EASY Card at any Transit Service Center by completing the EASY Card replacement form.

Fees:

• $5 for the first replacement
• $20 for the second replacement
• $50 for each replacement thereafter

A fee waiver is available when an original police report is presented listing the card as a stolen item.
